Kyphosis (Hunchback) Treatment and Spine Strengthening Guide with Clinical Pilates

Postural Kyphosis (commonly known as a Hunchback), which is frequently seen in growing adolescents due to the use of smart devices and in adults who constantly spend time at a desk or in front of a screen, is a condition where the shoulders slump forward and the natural curve in the thoracic spine becomes excessively rounded. This is not just an aesthetic problem that makes clothes fit poorly; it is also a serious mechanical disease that prevents the lungs from expanding fully, causing shortness of breath, and leads to chronic, aching back pain in the shoulder blades.

Why Does Kyphosis Occur and Why Simply Saying "Stand Straight" Doesn't Work?

Looking at your child or yourself in the mirror and constantly giving the command "Stand straight!" does not solve the problem. This is because, in postural kyphosis, there is a mechanical muscle imbalance that we call "Upper Crossed Syndrome" in physiotherapy.

Due to constant leaning forward, the chest muscles (Pectoral muscles) have shortened and become hard as a rock, violently pulling the shoulders forward. In contrast, the back muscles (Trapezius and Rhomboid muscles) that are supposed to keep the shoulders back have weakened, become exhausted, and stretched like a rubber band. Trying to hold the body upright with weak, exhausted muscles only lasts 1-2 minutes; the tired muscles give up, and the body returns to its old slouched posture.

Warning! Why Are Posture Corsets Sold Online Dangerous?

Many people resort to posture corsets that pull the shoulders and straighten the back as a shortcut. However, these corsets hold the body upright "passively." When the corset is worn, the already weak back muscles become completely lazy, thinking, "There is a mechanism holding me upright anyway, I don't need to work," and begin to waste away (atrophy). The moment you take the corset off in the evening, your slouching reaches a much worse state because your muscles are weaker than before. What we need is not a piece of cloth attached from the outside; it is a natural "inner corset" consisting of our own muscles built through exercise.

Our Posture Correction Strategy with Clinical Pilates in Marmaris

In our clinic, we solve kyphosis treatment not with random fitness movements, but with scientific physiotherapy and Clinical Pilates principles (based on the patient's posture analysis) in 3 stages:

  • Stretching the Chest Muscles (Myofascial Stretching): The first step is to release the torso. The short chest muscles (Pectoralis Major and Minor) that pull the shoulders forward are lengthened and opened with fascial release techniques and specific pilates exercises.
  • Back Strengthening (Clinical Equipment Pilates): Once the chest is opened, targeted strengthening of the weak back muscles that bring the shoulder blades (scapulae) together is achieved using resistance bands and the spring resistance of the "Reformer Pilates" machine.
  • Core Stabilization: By activating the deep abdominal and lower back (core) muscles that keep the spine—the main pillar of the body—unshakable and solid like a tree trunk, the upright posture is aimed to be "permanent."

Frequently Asked Questions (FAQ)

Is my kyphosis structural (bony) or postural?

If your back can become completely straight when you lie flat on a hard surface or when you force yourself, your problem is "Postural Kyphosis" caused by muscle imbalance and can be completely corrected with physiotherapy/pilates. However, if the bone curvature in the back does not straighten even when you lie down, this may be "Structural Kyphosis" (e.g., Scheuermann's Disease) and may require a brace or surgery under doctor supervision. This distinction is clearly made during the assessment in our clinic.

How long does it take for my posture to improve?

It takes time for muscles to elongate at the cellular level and strengthen by forming new bonds. With regular clinical pilates sessions and strictly following the prescribed home exercises, a visible, obvious posture improvement that can be noticed in photographs is usually achieved within 4 to 6 weeks.
