Manual Therapy Guide for Neck Hernia, Severe Headaches, and Nerve Compression

Physiotherapist Rumeysa Bingol Updated: September 2026

Working hunched over a computer for hours, looking down at a smartphone screen for extended periods (medically known as Text Neck Syndrome), and a high-stress lifestyle accumulating in the shoulders have led to an incredible increase in neck hernia (Cervical Disc Herniation) cases today. If you have persistent headaches, numbness radiating from your shoulder blades to your arms, and you wake up with a stiff neck in the mornings, the cervical (neck) region of your spine is signaling a need for urgent intervention by an expert physiotherapist.

How Does a Neck Hernia Form and Why Does It Affect the Arms?

Long hours spent leaning forward at a desk or looking at a phone shift the head's center of gravity forward, applying immense pressure to the cervical discs. This abnormal pressure crushes the discs between the neck vertebrae. When the outer wall of the disc (Annulus Fibrosus) tears and the fluid inside spills out, it puts severe pressure on the nerves (cervical nerve roots) extending from the spinal cord to the arms and hands. The most common neurological findings in our Marmaris physiotherapy patients who present with a hernia diagnosis are:

  • Sharp, burning pain radiating to the shoulders, back, and shoulder blades.
  • Numbness, tingling, or an electric shock sensation in the arms, hands, and especially the fingertips (Which finger is numb indicates the level of the hernia).
  • Waking up with severe neck stiffness (Acute Torticollis) in the mornings and being unable to turn the neck left or right.
  • Rock-hard muscle knots (Myofascial trigger points) in the back that don't go away, even with massage.
  • Ringing in the ears (tinnitus), dizziness, and tension-type headaches resembling migraines (Cervicogenic headaches).

Do Muscle Relaxants Treat a Hernia?

No. Patients often resort to muscle relaxants and painkillers as a temporary solution. However, these chemical methods only temporarily stop you from feeling pain by numbing the nervous system. A permanent recovery cannot be achieved without resolving the mechanical pressure of the cervical discs on the nerve, the misalignment of the vertebrae, and fascial adhesions (without eliminating the mechanical problem).

How to Achieve Neck Health with Manual Therapy?

The holistic approach we apply in our clinic in Marmaris or during our mobile home treatments is completely different from hooking a patient up to a machine and waiting. It involves side-effect-free and evidence-based manual therapy methods applied directly by the expert hands of a physiotherapist:

1. Joint Mobilization and Clinical Traction

Using manual therapy techniques, joint locks in the cervical vertebrae are released with gentle and absolutely painless maneuvers. Through specific traction techniques (safely pulling/extending the neck at a specific angle by hand), the compressed disc spaces are expanded. This procedure alleviates the destructive pressure and compression on the nerve roots within seconds, stopping the numbness radiating to the arm.

2. Fascial Release and Dry Needling

Focusing only on the bones is not enough. The tense muscle membranes (Fascia) that wrap the neck like armor and stubborn trigger points in the back are resolved with specific soft tissue manipulations. Natural cell regeneration is initiated by increasing the blood flow and oxygen to the area with Dry Needling when necessary.

3. Medical Exercise Prescription

To ensure that the neck, which has been opened and relaxed with manual therapy, does not return to its old bad posture two days later, highly specific medical exercise prescriptions designed to strengthen the deep neck muscles (deep cervical flexors) that hold the head upright are taught one-on-one to the patient.

Frequently Asked Questions (FAQ)

Is neck cracking or manipulation (Chiropractic) dangerous?

Harsh, uncontrolled, and sudden cracking movements performed by unqualified individuals (e.g., barbers, masseurs, or unspecialized bone-setters) are extremely dangerous; they can lead to severe spinal cord injuries, ranging from carotid artery tears to paralysis. However, clinical "Specific Joint Manipulation" techniques performed by a licensed physiotherapist trained in anatomy and biomechanics are completely safe, scientific, and instantly relieving.

Does wearing a neck collar cure a hernia?

According to current medical research, prolonged use of a neck collar (corset) is strongly discouraged. A neck collar causes the neck muscles to become lazy (atrophy). When you remove the collar, these weakened muscles cannot support your head, making your pain and hernia pressure more severe than before.
