Physiotherapy for TMJ (Jaw Joint) Pain and Teeth Grinding (Bruxism)
Do you wake up in the morning feeling severe fatigue in your jaw, cheeks, and temples, as if you've been chewing stones all night? Do you hear frightening "clicking or popping" sounds from your jaw when eating, yawning, or keeping your mouth open at the dentist? This condition, known as stress-induced nighttime teeth grinding (medically called Bruxism), is the biggest and most destructive indicator of a jaw joint disease called Temporomandibular Joint (TMJ) Dysfunction.
Why Does the Jaw Click? The Hidden Culprit: The Masseter Muscle
Our jaw joint (TMJ) connects the skull bone to the lower jaw bone and is one of the most complex and mobile joints in the body, enabling us to speak, chew, and swallow throughout the day. The "Masseter" muscle, responsible for chewing, is one of the strongest muscles in the human body relative to its size.
When this muscle involuntarily contracts throughout the night due to daily stress, anxiety, or sleep disorders (sometimes grinding the teeth together with a pressure exceeding 100 kilos), the delicate cartilage disc inside the joint gets crushed, deformed, and shifts forward. That 'click' or 'pop' sound you hear when opening and closing your mouth is the friction sound the bone makes as it jumps over that displaced cartilage. If this condition is not intervened with, "Jaw Lock" (Trismus) can occur over time, and the patient may not even be able to open their mouth a single inch.
Is Using Only a Night Guard (Splint) Enough?
The clear night guards provided by your dentist are an excellent and very necessary protector to prevent tooth enamel from wearing down, cracking, and breaking. However, wearing a guard does not stop your jaw muscles from contracting, nor does it stop the spasm signals the brain sends to those muscles all night long. Clinical manual physiotherapy is essential to mechanically release the knots of the Masseter and Temporalis (temple) muscles that go into spasm and become hard as a rock.
What Do We Do in TMJ (Jaw) Physiotherapy in Marmaris?
Jaw joint rehabilitation requires special expertise. Simply applying cream from the outside does not work. The protocol we carry out in our clinic, often in integration with dentists, is as follows:
- Intra-oral Manual Therapy: This is the gold standard of this treatment. Using gloves, the physiotherapist reaches the cheek muscles directly from inside the mouth (especially the Pterygoid muscles and the inner surface of the Masseter). The specific fascial release and massage techniques applied relieve the pain within seconds and increase the jaw's opening angle.
- Neck and Posture Alignment: Loss of cervical lordosis (neck straightening) or Forward Head Posture pulls the jaw joint directly downward and disrupts it. We manually relieve the tension in the neck muscles (especially the SCM and Trapezius muscles) and lighten the load on the jaw by adding posture exercises.
- Trigger Point Dry Needling: We find walnut-sized locked trigger points at the corner of the jaw and temples (Temporalis), and dissolve them within seconds using very fine, acupuncture-like needles, allowing the muscle to regain its former flexibility.
Frequently Asked Questions (FAQ)
Does intra-oral massage hurt?
It is normal to feel a slight tenderness (a "sweet pain") when the spasming muscles are first touched. However, as the muscle relaxes within seconds, this feeling is replaced by an incredible sense of relief and lightness.
Should I get Botox or Physiotherapy?
Masseter botox prevents contraction and stops teeth grinding by temporarily "freezing" the nerve transmission of the muscle (on average 4-6 months). Cosmetically, it also slims the jawline. However, when the botox wears off, the grinding problem returns exactly as before. Jaw physiotherapy can be done as a supportive measure alongside botox, or it serves as a mechanical, permanent, and natural solution for patients who do not want to be injected with chemicals.
