The Vital Importance of In-Home Physical Therapy After Fracture and Cast Removal

Physiotherapist Rumeysa Bingol Updated: September 2026

When a bone breaks as a result of trauma and the orthopedic physician places the area in a cast (or splint) for an extended period, the biological fusion of the bone usually takes 4 to 6 weeks. Almost all patients imagine that on the highly anticipated "day the cast is cut and removed," everything will return to normal, and they will be able to immediately get up and walk or use their arm. However, the scene they face the moment the cast comes off is often shocking: The leg or arm is thinned, the skin is dry and bruised, and the joint is locked as if turned to stone.

During this long period of immobility, the bone has fused, but the joint capsule and ligaments in the immobilized area have hardened, creating a contracture (joint locking), and the muscles have severely wasted away (atrophy) due to disuse. The real, challenging rehabilitation process begins that day.

Why Should Home Treatment Be Preferred After Cast Removal?

Especially in lower extremity (ankle, shinbone - tibia, knee, or hip) fractures, orthopedic physicians ask the patient not to put full weight on that leg for a while longer to protect the newly fused bone. During this period, going down stairs with crutches or a wheelchair, getting in and out of a car, and being transported to a clinic is very exhausting and risky for the patient. Thanks to mobile physiotherapy at home, the patient can start their treatment safely and quickly without facing the stress of the clinic and without putting the newly fused bone at risk of re-trauma (falling).

What Do We Do Step-by-Step in Post-Fracture Rehabilitation?

When our clinic's expert physiotherapists arrive at your home, they strictly follow the Weight Bearing protocols permitted by your doctor and take the following steps:

  • Regaining Range of Motion (ROM): Restoring the former natural flexibility of the joint that has been frozen in the cast for weeks (e.g., a stiff ankle) through orthopedic manual therapy techniques (Mulligan concept) and soft tissue stretching maneuvers.
  • Stubborn Edema Dispersion and Pain Management: When the limb is hung down after the cast is removed, it instantly begins to swell and throb. This stubborn edema is dispersed using specific lymphatic circulation massages, kinesiology taping, and elevation exercises.
  • Gradual Weight Bearing and Gait Training: Weight transfer exercises are performed strictly within the limits allowed by the doctor (e.g., 25%, 50%, or as tolerated) without ever damaging the fracture line. The patient is taught correct stepping with crutches (three-point gait, etc.).
  • Balance (Proprioception) and Muscle Strengthening Training: Wasted (atrophied) muscles are volumized again using clinical pilates bands. Brain-muscle reflexes are re-coded on special balance boards so that the joint does not sprain again.

Frequently Asked Questions (FAQ)

How long after the cast is removed should physical therapy start?

It should begin without wasting any time (immediately within the first 2-3 days) from the moment your orthopedic physician removes the cast, takes a control X-ray, and confirms that bone fusion is sufficient. Starting physical therapy late can lead to the tissue adhesions (contractures) becoming much harder, ossifying, and eventually requiring forced opening under anesthesia.

Will I have a lot of pain when the joint is being mobilized?

When mobilizing a joint that has been frozen for months, it is natural and necessary to experience a tolerable level of stretching pain due to the stretching of tissues. However, your physiotherapist will never do this at an intensity that would damage the bone or tear the muscle. Work is done within the physiological limits permitted by the tissue.
