The Vital Role of Home Physical Therapy in MS (Multiple Sclerosis)

Physiotherapist Rumeysa Bingol Updated: September 2026

Multiple Sclerosis (MS) is a chronic neurological disease that emerges when the protective shield (myelin sheath) around the nerve fibers connecting the brain and spinal cord is damaged by the patient's own immune system; it generally progresses with acute attacks and partial recoveries (remissions). The biggest complaints that make daily life difficult for MS patients are severe stiffening and contractions in the leg muscles (spasticity), balance loss while standing, and an inexplicable feeling of "Extreme Fatigue" that suddenly forces the patient into bed during the day.

After the Neurology Clinic: Why Should Home Treatment Be Preferred?

Physical therapy and rehabilitation are an inseparable half of the medical (medication/cortisone) treatment applied by the neurology doctor and MS management. However, by their nature, MS patients are incredibly sensitive to heat and excessive exertion. In medicine, this is called the "Uhthoff Phenomenon."

Especially during the summer months, in hot regions like Marmaris, or when the patient exerts themselves, if their core body temperature rises even by half a degree, their neurological symptoms (blurred vision, weakness in the arms, or balance loss) can suddenly worsen.

Based on this physiological reality; the effort an MS patient will expend outside in the heat to go to a clinic or hospital, the stress they will experience in traffic, or the fatigue they will feel while waiting in line for a machine in a crowded hall can reduce the entire healing efficiency of the physical therapy to zero. Home physical therapy (Mobile Physiotherapy) prevents the patient from wasting their limited "Golden Energy" on the roads and allows them to save this energy solely for healing and correct exercises.

What Are the Physiotherapy Strategies We Apply at Home in MS Treatment?

The affected brain region and complaints of every MS patient are as different from one another as a fingerprint. Therefore, "making everyone do the same standard exercise" does not work in MS treatment. Our clinic's mobile physiotherapists focus entirely on these personalized goals:

  • Extreme Fatigue Management: The living space is ergonomically arranged so that the patient can use their limited energy during the day correctly (energy conservation). Muscle-strengthening exercises are planned and given in a way that will never bring the patient to the point of exhaustion (with short sets and long rest intervals).
  • Spasticity (Muscle Stiffness) and Cramp Control: Painless passive stretches, correct joint positioning, and deep fascial release techniques are applied to relax the constantly contracting leg or arm muscles that disrupt sleep at night or make walking difficult.
  • Balance (Proprioception) and Coordination: To minimize the "drunk-like" stumbling experienced while walking and the life-threatening risk of falling, special neuromotor exercises based on deep sensation (proprioception) that strengthen the core muscles keeping the trunk upright are practiced.

Frequently Asked Questions (FAQ)

Is physical therapy performed during an MS "Attack (Flare-up)" period?

It must be strictly limited. During acute attack periods (that difficult phase when the patient falls into bed, neurological findings suddenly worsen, and they usually receive high-dose pulse cortisone treatment in the hospital), active and strenuous exercises that will further exhaust the patient are completely avoided. To prevent the joints from freezing during this sensitive period, only very light passive range of motion (ROM) exercises, requiring no effort from the patient, are performed. The main intensive and active physical therapy program is started after the attack subsides to quickly regain muscle losses.

Does exercising completely cure MS?

Multiple Sclerosis is a chronic disease and cannot be completely eradicated in the light of current medical knowledge. However, correct physiotherapy incredibly slows the progression of the disease (secondary muscle wasting and calcifications), and by keeping muscle functions alive during the periods between attacks, it prevents the patient from becoming dependent on a bed and a wheelchair. It is the greatest key to maintaining the quality of life.
